Minimum Qualifications Permanent qualifications: Candidates must currently be a permanent Program Aide
OR
be reachable on the current Civil Service eligible list.
Transfer qualifications: Candidates must be eligible for transfer under Section 70.1 of the Civil Service Law.
Provisional qualifications: Candidates must be a qualified employee of New York State and have had three months of permanent competitive or 55-b/55-c service in a G-5 or higher clerical, secretarial, keyboarding or paraprofessional** title set in an office environment. **Only paraprofessional titles in an office environment will be considered qualifying. Service in a direct care title is not qualifying experience. Examples of non-qualifying titles are: Developmental Disabilities Secure Care Treatment Aide 1; Direct Support Assistant; Food Service Worker 1; Licensed Practical Nurse; Mental Health Therapy Aide; and Security Services Assistant.
Duties Description POSITION DESCRIPTION: The Program Aide will report directly to the Quality Improvement Director and provide additional support to the Incident Management Unit in order to provide timely responses to the needs of the teams, investigators agency,(OIIA, and Justice Center), HR, Central Office, and any other appropriate entities (i.e. counsel's office, ERO), as it relates to incidents, regulations, protective measures, and keeping track of substantiated/founded targets for HR (for: staffing, promotions, admin leave report, etc.). Provide guidance to the teams in regard to incidents to ensure regulations are adhered to and agency is in compliance. The Incident Management Unit acts as a liaison for the various treatment teams to expedite the collection of critical data in order to complete required documentation for incidents within the required 24-hour time frame. Acts as an interface with Central Office Incident Management Unit and other state agencies regarding sensitive and confidential information. Extracts reports and trends from data bases and IRMA, develops spreadsheets for Incident Management Unit by securing information from various sources in order to track the status of incidents and investigations, and to provide reports on related areas of interest. Maintains computer and physical files (when necessary/applicable) of open and closed investigations. Prepares for, participates in, and follows up on weekly scheduled/routine meetings, such as Frequent and IRC to provide the appropriate tracking and oversight.
PRIMARY DUTIES AND RESPONSIBILITIES:
• Assists in preparation and review of documents for incidents.
• Liaises with various agency staff and other State, local and federal agencies.
• Ensures regulations are adhere to and ensures agency compliance with regulations.
• Maintains and updates database program (IRMA).
• Creates tables and charts and drafts reports for investigations.
• Completes the Annual Trends report (collects and analyzes data, creates tables and charts for the report).
• Follows up on incident documents from treatment teams.
• Supports communication with incident management, teams, and investigators.
• Files electronic and physical copies of supporting documentation.
• Maintain paper and or electronic files on open and closed investigations.
• Follow up on documentation requirements: initial documents, supporting documents, closing documents, and protective measures.
• Develops, maintains, and updates database programs and methods of data analysis via Incident Management Unit tracking spreadsheets.
• Disseminates incident notifications to teams (VPCR, 5-day reports, final investigated reports, Justice Center Letter of Determination, etc.).
• Completes meeting minutes as needed.
• Reviews and analyzes AOC/AOD and Triage reports for possible incident-related issues; reaches out to the teams for clarification.
• Follows up on deadlines for 5-day and 30-day updates, as well as internal final investigative reports.
• Assists in the development, review and/or updates to policies/procedures, as needed.
• Follows up with ongoing Plan of Corrective Action and Corrective Action Plan requirements.
• Assists with incident related DOH/DQI audit requests.
• Completes additional job tasks as assigned by supervisor.
Additional Comments OTHER: *Downstate Adjustment of $3,026.00 would be added to hiring rate/job rate for base is of Hudson Valley DDSO Thiells location (Rockland County).
