Minimum Qualifications MINIMUM QUALIFICATIONS:
1. You must take and pass a Civil Service examination in order to be considered for this position.
2. These positions are filled by the Select Job Titles portion of the Professional Careers Opportunities (PCO) examination.
3. To qualify for Health Facility Management Assistant, candidates must meet the following criteria:
• A bachelor's or higher degree.
• AND two years of professional administrative experience in a health-related facility.
PREFERRED REQUIREMENTS:
• Valid NYS driver’s License.
Duties Description POSITION DESCRIPTION:
The Health Facility Management Assistant will report directly to the Quality Improvement Director and support the department of Quality Improvement in the ongoing evaluation of services provided to individuals with developmental disabilities by state-operated facilities and programs.
Conduct studies to evaluate program needs, processes, outcomes, impacts, costs/benefits, effectiveness, and individuals’ satisfaction; collect, review, and analyze quantitative data related to programmatic services; produce management reports and recommend programmatic improvements based on analyses.
Assist in the development and modification of survey instruments and data collection systems; and develop, update, and maintain program evaluation protocols, policies, and procedures.
Duties may include, but are not limited to:
• Serve as a generalist within the department of Quality Improvement, supporting the day-to-day operations of each of its six units. Develop, implement, and analyze initiatives and all key performance indicators (KPIs) for every DDSOO department, the clinic, the 123 residential, and the 11-day habilitation state-operated programs.
• During weekly team meetings, identify trends with billing issues, service delivery, incidents, and operations. Develop measurable interventions using Lean concepts that include ongoing monitoring activities to ensure systems improvements.
• Develop trainings and guidance documents to reduce loss of revenue due to billing issues. Audit individual staff and programs to ensure successful implementation.
• Serve on the quarterly Quality Improvement Committee. Present on KPIs and the successfulness of systems improvements. If KPIs fall below acceptable thresholds, develop new interventions and ongoing monitoring activities to ensure compliance, successful billing, and achievement of the strategic plan and other department goals.
• Conduct monthly analysis of processes with low quality rates and develop operations guidance to improve outcomes, including reducing cycle time to receive documents in the Medicaid Compliance and Incident Management units, reducing the total amount of revenue loss due to self-disclosure, increasing the response rate to Satisfaction Surveys, and increasing attendance by individuals at Home and Community-Based Services meetings.
• Conduct at least four in-person site visits per month as part of recertification survey readiness efforts. Provide site-specific enhanced monitoring based on citations from previous audits, incident trends, and staffing concerns. Review newly implemented policies, procedures, and forms to assess effectiveness. Report training needs to the Habilitation Specialist Clinical Coordinator.
• Co-Chair the Policy and Procedure Committee, supporting efforts to standardize practices across both districts (Hudson Valley and Taconic). Manage weekly work groups to allow at least three departments to be updating a policy at the same time. Ensure that all regional policies for all departments and state-operated programs are reviewed every other year (or annually, when required).
• Maintain multiple Excel spreadsheets for the Director of Quality Improvement. On average, there are 15 spreadsheets with advanced reporting and alert capabilities that require daily input.
• Assist with development and maintenance of records for Agency Audits.
• Liaise with multiple teams and departments regarding Statements of Deficiency and Plans of Corrective Action every week.
• Serve as the backup for staff responsibilities related to Plans of Corrective Action.
• Serve as the backup for staff responsibilities related to Medicaid Compliance.
• Create and update SharePoint sites and Teams channels.
Additional Comments OTHER: *Downstate Adjustment of $3,026.00 would be added to hiring rate/job rate for base is of Hudson Valley DDSO Thiells location (Rockland County).
