Job Specifics

Duties Description The Bureau of Social Care and Community Supports works to integrate health and human services within the Medicaid program. The bureau is composed of two units, Supportive Housing and Social Determinants of Health. This position will work under the Social Determinants of Health Unit.
This position is responsible for the strategic oversight, management, and operational excellence of the 1115 Waiver Social Care Network (SCN) program. This role will lead a team of Contract Managers, ensuring effective relationships with SCN providers, fiscal administration, compliance, and the achievement of program goals. The position will play a key role in policy development, stakeholder engagement, and reporting to senior leadership.

Key Responsibilities:

Team Leadership and Development:
-Supervise, mentor, and train Contract Managers and other program staff.
-Foster a collaborative and high-performing team environment.
-Conduct performance reviews and support professional development.

Provider Network Management:
-Oversee the management of contracts with Social Care Network providers.
-Ensure effective communication and relationship building with SCN partners.
-Track SCN milestones and performance against contractual obligations.

Fiscal Administration and Compliance:
-Oversee the fiscal administration, including payments and funding distribution, to Social Care Networks.
-Ensure compliance with all relevant regulations, policies, and contractual terms.
-Monitor program budgets and financial performance.

Reporting and Performance Monitoring:
-Develop and implement robust reporting mechanisms to track program progress and outcomes.
-Prepare and present regular reports to senior leadership on SCN program performance, challenges, and achievements.
-Analyze data to identify trends, areas for improvement, and opportunities.

Strategic Planning and Policy:
-Contribute to the development and refinement of organizational policies related to social care networks.
-Participate in strategic discussions to align SCN programs with organizational objectives.
-Develop and implement strategies to achieve program goals and enhance service delivery.

Stakeholder Engagement and Communication:
-Lead meetings and technical assistance sessions for SCN providers and internal teams.
-Plan and develop educational webinars and training materials for providers and staff.
-Present program updates and findings to diverse stakeholder groups, including community partners, government agencies, and leadership.
-Collaborate with internal units and teams to develop and implement relevant processes and procedures.

Minimum Qualifications "Reassignment: Current NYS Department of Health employee with permanent or contingent-permanent, competitive status as a Health Systems Specialist 3, G-23.

Permanent Transfer Candidate: Current NYS employee with one of year of permanent or contingent permanent, competitive service in a title Grade 21 or higher deemed eligible for transfer to Health Systems Specialist 3 under Section 70.1 or Section 52.6 of civil service law.

Preferred Qualifications: Program or project implementation and management experience

Additional Comments This position may require travel throughout New York State including overnights to areas not served by public transportation.
