Job Specifics
Duties Description The Habilitation Specialist 2 (HS2) will oversee the various programs in the program center. Each program area requires oversight of a Habilitation Specialist to ensure services are provided for all individuals based on their needs and valued outcomes and to directly supervise./provide guidance to the HS1s in the program center. This position has a current caseload of 10.
Duties include, but are not limited to:
• Provide supervision and guidance to the HS1s assigned to the program center.
• Writing and updating Staff Action Plans at least semi-annually to reflect current Lifeplan goals and needed services.
• Reviews all monthly summary notes.
• Ensure service delivery compliance, data collection forms, monthly summary notes and billing documentation are all completed in a timely manner.
• Provide opportunities for socialization to specialized populations and high behavior needs individuals who cannot be served by a voluntary agency.
• Responsible for the health and safety of the individuals participating in the day habilitation program.
• Ensure the direct support staff complete the appropriate documentation for the individuals they are directly serving.
• Other duties as requested by the supervisor.

Minimum Qualifications Promotion:
One year of service as a Habilitation Specialist 1 and reachable for appointment on the current Civil Service Eligible List for this title.
Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

*Preferred Qualifications: QIDP (Qualified Intellectual Disorder Professional) requires one year's experience working with people with a developmental disability AND a bachelor’s degree or higher in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling or psychology).
Additional Comments • Please note that starting salary for this title reflects the Mid-Hudson adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 17 ($63,361) and Mid-Hudson adjustment ($2,000). Mid-Hudson adjustment is also added to the job rate ($80,655).
• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 or 52.6 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
