Minimum Qualifications MINIMUM QUALIFICATIONS:

Currently a permanent Habilitation Specialist I (who meets QIDP standards)

70.1 Transfer Quals:
One year of permanent service in a direct care/paraprofessional/or professional title - Grade 12 or higher (who meets QIDP standards).

Open Competitive:
Persons who are on the current Habilitation Specialist I Civil Service list may also apply. Consideration will be
contingent upon reachability or eligibility in accordance with Civil Service Law.
Provisional:

If appointed on provisional status, you must participate in the next Civil Service examination(s) and be a reachable candidate in order to obtain permanent status and maintain your position.
For the Transition Examination, you must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had:

Three months of permanent competitive or 55 b/55 c service in a clinical or direct care position. The list of qualifying titles is available at: http://www.cs.ny.gov/examannouncements/announcements/00234titles.cfm; AND
You must also possess a bachelor’s degree or higher in a human services field* OR permanent New York State certification as a teacher.

For the Open-Competitive Examination, you must have a bachelor's or higher level degree in a human services field* AND one year of direct care or clinical experience where your primary focus is providing services to individuals with developmental disabilities.

Substitution: A master's degree in a human services field* OR permanent New York State certification as a teacher may be substituted for the degree and experience described above.

Duties Description POSITION DESCRIPTION:

Ensure individuals reach their optimal level of functioning by developing individual day habilitation plans, setting goals, and monitoring an individual’s progress in reaching those goals. Provides clinical guidance to, and may supervise a limited number of direct care staff.
PRIMARY DUTIES AND RESPONSIBILITIES:

• Development of program plans based on each individual’s valued outcomes.
• Documentation of program-specific plans.
• Participates in individuals’ ISP meetings.
• Participates in activities which support the program(s) and/or the unit’s approach to providing optimum care for all individuals.
• Oversee all aspects of special components of the program which occur regularly.
• Verify all program services are provided as prescribed by team planning process.

* For the purpose of these examinations, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling, education, human development.)
