Job Specifics
Minimum Qualifications Applicant must be an employee of The Office for People with Developmental Disabilities and must have permanent or contingent permanent status as a Developmental Disabilities Program Specialist 1; OR be reachable on the Civil Service list; OR currently encumber a position in which you are eligible to transfer to this position.
FOR PROVISIONAL APPOINTMENT: One year of service in a position at Grade 16 or above within OPWDD.
Duties Description The incumbent will be the Coordinator of the Bronx Self Direction Unit. The duties will include but will not be limited to:
• Oversee the development of a self-directed plan and
amendments. Work closely with the participant, family members, Care Managers, broker and fiscal intermediary agency.
• Review plans for accuracy; complete necessary paperwork for approval. Submit plans electronically to Central Office. Once final approval is obtained, send approval letters and other correspondence to all parties.
• Provide technical support to brokers (start up and support brokers), Care Managers and all involved in the development and maintenance of a self-directed plan.
• Participate in “launch meetings” when the plan is approved. Ensure all parties are aware of their responsibilities and provide guidance on the implementation of the plan.
• Participate in ongoing statewide workgroups and trainings for brokers and other interested parties. Work with other self-directed liaisons regionally to provide trainings for new brokers. Track on-going training requirements for brokers.
• Provide person centered planning information as needed to interested Care Managers, parents, brokers, etc. Provide information regarding OPWDD’s transformation and any upcoming changes within self-direction. Participate in forums and presentations to agencies and parent groups interested in self-direction.
• Maintain data based tracking of interested individuals, the status of the plans and review dates.
• Approve DDP1 in CHOICES and maintain additions and withdrawals as they occur.
• Supervise the Self Direction Team, oversee time and attendance, provide training and guidance.
• Conducts Bi-monthly SD info Sessions for individuals and families.
• CHOICES liaison for Metro Bronx DDRO
• Utilize the Development Plan for SD cases as needed
• Obtain program codes for newly approved FI and Brokers.
