Minimum Qualifications: You must be a qualified employee of the New York State Office for People with Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service in a position at a Grade 16 or above.

• Ability to make independent decisions when supervisor is unavailable.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Sunmount DDSO Assistant Family Care Coordinator is responsible for the day-to-day operational fiscal and regulatory management of a current 35 home 77-person capacity program spread throughout the six county district. This position includes significant travel.
Specific responsibilities include:
• Collaborate with Family Care Coordinator to ensure compliance with written policies and procedures relative to Family Care Home Certification and Recertification. Assist Family Care Coordinator with review and writing of policy and procedure.
• Ensure the Certification and Recertification process is completed in a timely manner.
• Coordinate, develop and facilitate training. Ensure that each prospective Family Care provider receives required training prior to issuance of the operating certificate and that once certified, mandatory training is completed yearly. Ensure all providers are trained regarding Family Care rules, regulations and policies and have a knowledge of the Family Care Manual including use of required forms and documentation.
• Review Family Care Home Evaluation and Survey Inspections, verifying that deficiencies are addressed through a Plan of Correction in a timely manner. Collaborate with Safety Officer and Home Liaison in regards to achieving and maintaining Family Care home environment to set regulation.
• Review and track Family Care Home Liaison monthly visit to the Family Care home.
• Work collaboratively with Nurse Administrator to ensure that quarterly nursing visit to the Family Care home is completed.
• Ensure that a current record is maintained on each Family Care provider.
• Collaborate with Family Care Coordinator to complete Annual Visits which includes site visits to Family Care home and chart reviews at various offices.
• Working with the DDSO Business Office, monitoring that payments for home, provider, and other reimbursements are made in a timely manner. Working through Acumatica to ensure that Room & Board payment and personal allowance is appropriately allotted monthly. Monitoring that monthly Difficulty of Care Payment is processed appropriately and Off Line Payments are generated as necessary.
• Monitoring and approving respite services, trial visits, sitter services and therapeutic leave.
• Ensuring that the Staff Action Plan contains appropriate content and that the goals, supports, tasks, and preferences of the individual as outlined in the Plan are being implemented.
• Serving as liaison with the DDSO Quality Management regarding temporary suspension of operating certificates, home closure, application denial, and hearings.
• Extensive travel to Family Care homes to attend meetings/address issues. Regular visits to DDSO offices to provide staff support, attend meetings and complete chart reviews.
• Directly supervise Family Care Home Liaison staff.
• Provide training to ensure staff are knowledgeable of regulations and policies, including new policy as implemented.
• Coordinate and develop agenda for Staff Meetings.
• Additional duties as assigned by the Family Care Coordinator.
