Minimum Qualifications • Must currently be permanent in the title of Social Work Assistant 3, OR

• Must be active and reachable on the appropriate Civil Service eligible list, OR

• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.

FOR PROVISIONAL APPOINTMENT:
• Must possess a Master’s degree in Social Work from a regionally accredited college or university, OR

• A Bachelor’s degree in Social Work and one year of post bachelor’s experience in Social Work, OR

• A Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.

If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained*

Duties Description The incumbent’s job duties will include, but not limited to the following:

1. Serve as the primary staff person in managing regional intake and distribution of residential referrals

2. Review referral packets for completeness and follow up with referring party as necessary

3. Log referrals in excel spreadsheet and certified residential opportunities database

4. Review all referrals to ensure individuals are being served in the most community-inclusive setting and that all other services have been considered in lesser restrictive settings

5. Produce and distribute emergency needs list

6. Send out referral materials to provider agencies as requested

7. Manage the regional referral mailbox

8. Record all vacancies reported by provider agencies, follow up as necessary

9. Assist with referrals and follow up as requested

10. Work collaboratively with capacity management team and deputy director

11. Produce monthly reports, and ad-hoc reports as requested

Additional Comments SALARY INCLUDES $3,026 LOCATION PAY
