Job Specifics

Minimum Qualifications MINIMUM QUALIFICATIONS:

• Permanent, in title LPN
OR
• Licensed and Registered as an LPN in NYS

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Medical nursing treatment as necessary
• Coordinate, schedule, participate in and follow up on all medical and dental appointments
• Order and/or reorder all medications and medical supplies weekly. Transcribe all MD orders and maintain MD orders book
• Res hab activities, ADL's, appointments, etc., as directed by house supervisor, and any other tasks assigned to a Direct Support Assistant
• Direct care duties as assigned

Additional Comments ADDITIONAL INFORMATION:
• If you are employed by state government you may be able to receive loan forgiveness under the Public Service Loan Forgiveness Program (PSLF). The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
