Minimum Qualifications For permanent appointment, candidate must be currently employed by New York State as a Habilitation Specialist 1 with permanent or contingent permanent service and eligible for transfer, OR
Currently be employed by New York State and reachable for appointment on the Civil Service list for this title.
Duties Description The Habilitation Specialist 1 provides a variety of activities in support of our individuals diagnosed with intellectual and/or developmental disabilities. Habilitation Specialist are responsible for developing/assisting with individualized person-centered plans, communication, and appropriate social/community skills. The Habilitation Specialist 1 emphasizes the importance of community integration through community outings and volunteer training opportunities.
• Develop and implement Staff Action Plan with goals identified in individual’s Person-Centered Life Plan.
• Provide assistance and skill development training to individuals in a manner that maximizes personal choice and rights.
• Attend Annual and Semi-Annual Person-Centered Life Plan Meetings as scheduled.
• Daily monitoring of data entrée, service compliance and community inclusions.
• Complete Monthly Summary Notes and Group Day Hab Data Checklist, and document persons’ progress towards Life Plans goals.
• Implement Safeguard Summary Plans, Behavior Support Plans, Life Plans, as well as other treatment plans, and train direct support staff on adherence to established treatment plans.
• Closely monitoring records books to ensure compliance.
• Create monthly activity calendar with meaningful activities for individuals within their community.
• Serve as a liaison between agency, Care Coordination Organization/Care Coordinators (CCO), and families, and establish a working relationship to foster the delivery of care.
• Provide transportation to and from community activities and services.
• Participate in scheduled online and in-person trainings, as well as scheduled in-services.
• Additional duties as assigned.
