Duties Description POSITION DESCRIPTION:
Regional Office 4 is seeking an Aging Out Supervisor to oversee the Residential School Transition Coordinators who plan for individuals in residential schools and in ACS custody. The Aging Out unit is part of the Housing Division in Region 4.
Beyond the role of an AO coordinator, this staff person will train staff on the DDP2 assessment and serve as the high needs funding expert for the team.
PRIMARY DUTIES AND RESPONSIBILITIES:
• Function as a line supervisor for the Age Out Unit.
• Ensure that individuals aging out of residential schools receive appropriate OPWDD adult services in a timely fashion. Bring any issues and concerns to leadership.
• Assist with monitoring the integrity and accuracy of Aging Out Database data including running monitoring reports and working with the Aging Out Regional Supervisor to ensure data is being entered in a timely fashion.
• Assist with data management and reporting for individuals transitioning from residential schools to OPWDD adult services, using Excel and Citrix
• Be the DDP2 assessment point of contact for Aging Out staff, being able to answer questions, provide training, and technical assistance when necessary.
• Follow the Front Door process including preparing the DDP2
• Serve as the AO team’s higher needs funding knowledge expert, interfacing with the higher needs funding committee coordinator as necessary.
• Participate in weekly and monthly meetings, both in person, by phone or by video conference with agencies, families, residential schools and developmental staff, all meetings relate to the transition of individuals from residential settings to OPWDD adult services
• Review Preliminary Adult Services Plan/Behavioral Plan to ensure the needs of the individual will be met by the OPWDD Adult Residential Opportunity. If the plan is not approved, work with the agency to make corrections, deletions, or additions.
• Visit Residential Schools both in state and out of state for upcoming graduates as needed
• Maintain dialog with families, both in writing and over the phone regarding their child and the placement process.
