Minimum Qualifications PROMOTION: Candidate must have one year of permanent competitive service as a Habilitation Specialist 1
OR
be currently employed by New York State as a Habilitation Specialist 2 with permanent or contingent permanent service and eligible for transfer;
OR
be reachable for appointment on the Civil Service list for this title.
Duties Description The incumbent’s job duties will include, but not limited to the following:
• Develop Residential Habilitation Plans as reflected in the ISP as well as the needs and interests of assigned individuals
• Provide guidance/training to staff in all areas related to Residential Habilitation
• Monitor the implementation of habilitation plans and revise as needed
• Participate in Individual’s ISP meetings
• Ensure Residential Habilitation documentation is properly filed and neatly maintained as per regulation and policy
• Participate in the planning and implementation of in-house activities schedule as well as community inclusion activities
• Ensure a Personal Expenditure Plan is developed for each individual
• Verify all program services are provided as prescribed by the team
• Complete incident investigations as assigned
• Complete specialized assignments as assigned by the Treatment Team Leader
