Minimum Qualifications Reachable on the Civil Service List for Habilitation Specialist 1 OR

Permanent service as a Habilitation Specialist 1 OR

Eligible to 70.1 transfer to a Habilitation Specialist 1 title OR

Eligible for provisional appointment pending examination. To be eligible for provisional appointment, candidates must possess a bachelor’s degree or higher in a human services field and have one year of permanent competitive or 55-b/55-c service in a clinical or direct care position providing services to individuals with developmental disabilities.

* A degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

* If you are appointed provisionally, you must take the next scheduled Civil Service examination for this title and your score must be reachable ton the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained.*

Duties Description The incumbent’s job duties will include the following:

• Develop and implement Staff Action Plan with goals identified in individual’s Person-Centered Life Plan.

• Record/review/maintain Home and Community Base Service (HCBS) documentation monthly (Monthly Summary Notes and Face to Face Residential Service Checklist).

• Develop/Review/Revise Safeguard Summary Plans (SSP) as needed.

• Attend Annual and Semi-Annual Person-Centered Life Plan Meetings as scheduled.

• Ensure all Residential Habilitation HBSC documents are filed in the individual’s records, as per OPWDD order of filling regulations and policy.

• Provide ongoing training to direct support staff in all areas related to Residential Habilitation.

• Provide oversight to assigned homes and bring issues/concerns to the attention of appropriate team members.

• Assist individuals to develop and maintain relationships

• Serve as a liaison between agency, Care Coordination Organization/Care Coordinators (CCO), and families, and establish a working relationship to foster the delivery of care.

• Provide input to the House Manager or Recreation staff in the planning of community outing activities that are appropriate to each individual’s interests and abilities.

• Provide transportation to and from community activities and services.

• Participate in scheduled online and in-person trainings, as well as scheduled in-services.

• Additional duties as assigned.
