1. Permanent service as a Licensed Master Social Worker 2

The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

In the event there is not a viable eligible list, candidates that meet the following qualifications may be qualified for Provisional Appointment:
Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience;**
Or B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).
** Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.
Duties Description POSITION DESCRIPTION:
The selected candidate will serve as a vital team member of the Office for People with Developmental Disabilities (OPWDD) Region 1 Regional Office. The LMSW2 will provide social work supports within the region both functionally and geographically to ensure consistent delivery of key Regional Office services within the 17 counties. Key functions in the Regional Office include but are not limited to: Eligibility Review and Determinations; Service Assessment, Planning and Authorization; Fiscal and Programmatic Oversight. Social work supports are provided to all of the functional teams within the Regional Office which includes: Front Door, Waiver Services, Eligibility, Self-Direction, Housing, Crisis, Property/ Development, Fiscal, and Quality Management/Technical Assistance. Occasional evening and/or weekend work may be required. Occasional travel within the 17-county region is required.
PRIMARY DUTIES AND RESPONSIBILITIES:
Specific responsibilities will include, but are not limited to:
1. Serve as a member of Region 1 Regional Office providing the full range of social work services to support individuals and families.
2. Serve as the Intensive Treatment Program (ITP) Liaison; Assist the ITP referral source with processing a complete and accurate referral application.
3. Participate in initial 14-day ITP treatment plan meetings and routine ITP meetings with key stakeholders to monitor the child’s progress and discharge plan.
4. Facilitate linking children preparing to discharge from ITP back to enrollment into CCO Care Management, OPWDD services and appropriate Educational (school) services; Complete level of care determination (LCED) to support CCO and OPWDD enrollment.
5. Maintain communication with key stakeholders, including staff on various Regional Office teams, Central Office, Care Management agencies and Voluntary Provider agencies.
6. Serve as a liaison between the Regional Office, CCO providers, Cross System agencies, OPWDD Provider agencies, Individuals and Families to ensure comprehensive service planning.
7. Serve as the Child Protective Services Liaison.
8. Assist with processing Intensive Behavior Service referrals and completion of assessments.
9. Additional duties to provide support to the Crisis Team may be assigned as needed.
10. Additional duties may also be assigned to assist in supporting Regional Office operations.
11. Supervision and training of subordinate staff may also be required.
