Minimum Qualifications MINIMUM QUALIFICATIONS:

• Must currently be permanent in the title of Social Worker Assistant 3, OR

• Must be active and reachable on the appropriate Civil Service eligible list, OR

• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.

FOR PROVISIONAL APPOINTMENT

• Must possess a Master’s degree in Social Work from a regionally accredited college or university, OR

• A Bachelor’s degree in Social Work and one year of post bachelor’s experience in Social Work, OR

• A Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.

Duties Description The following responsibilities of this position are essential to ensure the health and safety of the individuals that OPWDD’s community-based providers serve and to ensure timely payments:

• Provide clinical oversight for environmental modifications (E-Mods), van modifications projects (V-Mods), and assistive technology for HCBS waiver enrolled individual

• Liaison with other units within OPWDD Regional Office to verify eligibility and waiver enrollment status

• Review and approve environmental modification request documents; ISP, Life Plans, LCED, Medical/Clinical Justification, Proof of home ownership, and proof of vehicle ownership

• Correspond with Care Managers and Families to discuss the overall process, and review required documents

• Compile and submit request packets for approval to DOH and OPWDD

• Liaison with OPWDD voluntary agencies for processing of environmental modifications under new process

o Send approved paperwork for modifications
o Review home visits and scopes of work, Review bids
for accuracy, Review final reports upon completion

• Participate in monthly/quarterly meetings with OPWDD Regional Office staff, and Care Managers to provide updates/overview of E-Mod/V-Mod process

• Coordinate meetings with voluntary agencies who have E-Mods/V-Mods contracts.

• Troubleshoot issues that arise throughout the E-Mods process.

• Correspondence with Central Office, agencies, Care Managers, families, and contractors throughout the process (written and verbal.

• Conduct site visits as needed, to review E-Mods project process

SELECTION CRITERIA:

• Possess excellent and professional communication, public speaking, writing and organizational skills;

• Ability to initiate and maintain working relationships with others such as families/individuals, advocates, community representatives, voluntary providers, DDRO, DDSOO, contractors and other agency staff;

• Computer knowledge and ability to maintain databases and monitoring systems;

• Ability to work independently and as part of a team;

• Possess acceptable time and attendance record.
