Minimum Qualifications Open Competitive:
A Bachelors Degree in a human service field and two years of professional experience providing vocational, psychosocial or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with mental retardation, developmental disabilities, mental illness or addictions. A Masters Degree in a human service field may substitute for one year of the qualifying experience. A Masters Degree in rehabilitation counseling may substitute for two years of the qualifying experience.
Promotion:
Two years of permanent service as a Rehabilitation Assistant 2, Occupational Therapy Assistant 2, or Physical Therapy Assistant 2 and a bachelor’s degree in a human service field or NYS licensure as a Registered Nurse; or one year as a Rehabilitation Assistant 2, Occupational Therapy Assistant 2, or Physical Therapy Assistant 2 and a master’s degree in a human service field.

Candidates must be reachable for appointment on the current Civil Service Eligible List OR; candidates must have permanent competitive status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4 or 52.6 of Civil Service Law.

Provisional appointment may be considered. Promotion: To be eligible, candidate must be a qualified employee of New York State and have two years of permanent competitive or 55-b/55-c service as a Rehabilitation Assistant 2, Rehabilitation Assistant 2 (Spanish Language), Occupational Therapy Assistant 2, or Physical Therapy Assistant 2 AND a bachelor’s degree in a human service field or NYS licensure as a Registered Nurse; OR one year as a Rehabilitation Assistant 2, Rehabilitation Assistant 2 (Spanish Language), Occupational Therapy Assistant 2, or Physical Therapy Assistant 2 AND a master’s degree in a human service field.
Open Competitive: To be eligible, candidates must possess a bachelor’s degree in a human service field AND two years of post-bachelor’s qualifying experience; OR a master’s degree in a human service field AND one year of post-bachelor’s qualifying experience; OR a master’s degree in rehabilitation counseling.

Duties Description 1. Develop and implement active treatment programs in the areas of skills development such as but not limited to vocational, substance abuse and restoration for individuals as it relates to lifestyle improvements, work and employment.

2. Develops and implements appropriate and innovative activities and curriculum implementation that support active treatment skills acquisition in a variety of settings, including but not limited to the classroom, the residence, the community, classroom-without-walls opportunities and so forth.

3. Ensure implementation, documentation of and Billing for Day Program Services.

4. Compliance with Regulatory and billing requirements and with agency policies and expectations.

5. Provide Day to Day supervision of and operation of Day Program site (including daily staffing) along with Hab Spec 2 and DSP supervisor.

6. Ensures Compliance with Program budget and fiscal management procedures.

7. Ensures compliance with various agencies’ regulatory oversight relevant to active treatment training programs (Dept. of Labor, OSHA, Dept. of Health, etc.).

8. Have Knowledge of Willow brook reporting/documentation requirements.

9. Adhere to the incident reporting and notable event processes.

10. The Rehabilitation Counselor 1 will also train staff to the specific tasks to be performed and provide appropriate guidelines; Train and supervise staff in appropriate reactions to emergency or crisis situations; Evaluate and schedule staff training sessions as mandated; Participate, with other members, in the development of Individuals’ habilitation plans; Ensure a safe and healthy environment for the individuals we serve; Support the core mission of the Agency.

11. In conjunction with other supervisors, the Rehabilitation Specialist 1 will engage in supervision of staff to ensure implementation of Behavior Support Plans, and other plans, services, and activities as outlined in the Life Plan.

12. Whenever possible, attends and participates in individuals’ Life Plan meetings (annual, semi, special) and various other meetings regarding individuals’ services/programs and team
concerns.

13. As specified by regulations, facility standards and policies, does the following:
• Completes evaluations and written assessments with accurate findings, strengths, needs, objectives and recommendations for addressing identified needs.
• Develops and implements program goals, revising as needed.
• Models correct implementation of program goals.
• Collects, monitors, analyzes, and summarizes data – writing monthly and quarterly notes.

14. Prepares and submits a wide variety of administrative reports including but not limited to: incident reports, Business Office, and Human Resources Office policy and procedures, OGS/Vehicle Maintenance, Staff services requirements.

15. Works as a member of the interdisciplinary treatment team.

16. The incumbent will supervise some other specialists who are involved in various functions in the Day Program which are all geared towards the care, treatment, and habilitation services of the Individuals we serve at Brooklyn Day Habilitation Program.

17. The incumbent will oversee the financial operations of the Day Program and liaise with the Revenue Support unit in all affairs concerning Individuals in our Day Program.

18. In addition, serves as liaison with the Residences, Care Coordinators, DDRO, and other arms of our operations.

19. Other duties as assigned.
