Minimum Qualifications Candidates must be employed by New York State to be eligible to apply for this position.
Candidates must have permanent status as a Mental Health Program Specialist 3, Grade 25 and be eligible for lateral transfer; OR one year of permanent, competitive service in a title eligible for transfer via Section 70.1 or Section 52.6 of the Civil Service Law.
Based on federal and/or state regulations, this position requires vaccination against COVID-19. Upon appointment, the selected candidate must be fully vaccinated, or willing to become fully vaccinated, against COVID-19 and will be required to provide proof of vaccination. Candidates who are not fully vaccinated against COVID-19 at the time of interview may not start working until two weeks after their final dose of a COVID-19 vaccine.
Duties Description The Office of Mental Health (OMH) is seeking a qualified candidate to fill a Mental Health Program Specialist 3, G-25 position under the direction of the Adult Home Settlement Unit Supervisor for Community Integration. The incumbent of this position will provide oversight and implementation of one of the high profile Olmstead Act related projects, the Adult Home Settlement Initiative, which has a goal of assisting high risk/high need individuals in transitioning to, and maintaining, residence in community settings.
Specific duties include, but are not limited to, the following:
• Oversee the subset of class members transtioned into OMH funded housing. This group of class members have moved out of DOH Adult Homes and into their own supported apartments or into OMH licensed housing.
• Supervise subordinate staff. Assign and review work. Conduct performance evaluations. Ensure Adult Home Settlement staff are provided training necessary to perform their duties and respond timely to all post transition issues/concerns, elevate any concerns to Unit Supervisor that require escalation.
• Review and produce reports on trends/findings seen in class members that transitioned to the community. Monitor program findings and make recommendations to Unit Supervisor. Monitor agency progress to make sure changes are implemented or improvements are made.
• Reviews, edits and finalizes documents and materials, including monthly and quarterly reports that demonstrate achieving improvements on process and transition metrics, housing placement progress over time.
• Communicate weekly with OMH and DOH staff, other agencies and providers to track progress made.
• Participate in quarterly leadership calls and meetings with OMH and DOH settlement staff.
• Manage and track Post Incident Reports. Responsible for timely submission of post incident reporting to the Court, plaintiffs, independent reviewer. Promote technology and evidence based practices that will assist former adult home residents to remain in community settings permanently.
• Coordinate and gather necessary information to any post transition inquiries or concerns. Draft responses sent and oversee follow up on the inquiries.
• Participate and assist to coordinate all of the Full Court Press and Housing Fairs under the direction of the unit supervisor of the Community Integration.
• Promote technology and evidence based practices that will assist former adult home residents to remain in community settings permanently.
• Assist the Adult Home Settlement unit supervisor as needed with Settlement Decision Date requirements, including notification to class members.
