Duties Description Overview
Regional Field Office (RFO) 4 is seeking an DDPS2 for the housing unit. The incumbent will have a range of responsibilities related to 1) High Needs Funding, 2) non-certified housing, and 3) placement for individuals with complex needs. The housing unit encompasses the aging out (residential school transition), certified residential opportunities, and non-certified housing. The DDPS2 will work collaboratively with teams with the housing unit and across the other functional areas including crisis, front door, and fiscal.

Duties Include but not limited to:
High Needs Funding (HNF)

Monitor the HNF mailbox for provider requests

HNF acuity level 1, 2, and requests:
• Review clinical documentation submitted by providers to ensure proper justification was provided in accordance with the HNF methodology/policy
• Interface with providers to request more information if necessary
• Ensure the approval of acuity level 1 and 2 requests timely
• Update tracking systems and verify HNF enrollment
• As necessary, provide agencies with education/information re: the HNF referral and approval process.

Manage HNF acuity level 3 requests
• Review materials submitted in provider’s HNF acuity level 3 referral and request additional information as necessary.
• The DDSP2 will review clinical documentation and the provider’s plan to serve the individual with extra funding with an understanding of the justification and information required in the BSPs (including fading plan), FBAs, and other required information and request revisions to the agency plans when necessary.
• Facilitate the RFO HNF review committee meetings and bring additional questions or requests for information back to the referring agency ensuring the committee receives the necessary information timely so the referral can be advanced to central office for review
• Interface with central office re: applications and additional information that may be required, following up with the referring agency and obtaining the information timely
• Notify agencies of approvals and ensure HNF enrollment
• Update tracking systems

Rental Subsidies (non-certified housing)
• Supervise staff responsible for conceptual approvals
• Ensure part 1 requests are processed timely and databases/tracking systems updated
• Monitor the quality of reviews and data reporting
• Ensure reports are accurate and provided to central office timely
• Manage additional responsibilities associated with non-certified housing as regional field offices statewide are making adjustments to their processes to ensure consistency.

Placement for Individuals with Complex Needs
• Manage the special review process for individuals transitioning from the ITO to the community or for those who have been unsuccessful in obtaining placement offers in other settings.
• With an understanding of the individual’s needs and residential referral history, identify barriers to placement in the clinical materials (i.e. referral packet) and make recommendations regarding how the materials may be updated to including such things as strength-based language, behavioral data that better reflects progress, resolving inconsistencies such as inaccurate diagnosis, medical conditions, or misrepresentations that need to be addressed in an updated Life Plan, addendums, new clinical summaries etc.
• Work collaboratively with the capacity management and crisis teams who will be a part of the review process described above
