Minimum Qualifications 1. Permanent service as a Medical Specialist 1.

OR

2. Eligible to 70.1 or 52.6 transfer by way of current permanent status in a title deemed comparable to Medical Specialist 1, by the Department of Civil Service.

OR

2. Possession of a license and current registration to practice medicine in New York State AND successful completion of a residency training program in your specialty approved by the Accreditation Council for Graduate Medical Education (ACGME).
*Limited Permit Option: If you are licensed in another state or by written examination in Canada, AND meet the other requirements for Medical Specialist 1, you are also eligible if you have a current limited permit to practice medicine in New York State. If appointed, you must obtain your license within one year of the date your limited permit was issued. Failure to do so will result in removal from your position.



Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Medical Specialist 1 provides primary medical care, working in collaboration with other physician(s) and nurse practitioners or midlevel providers for adults with developmental disabilities and co-occurring conditions (ie: mental health). Services are provided primarily through the Adirondack Clinic.
Duties would include, but not be limited to review of medical histories, perform physical examinations and manage the medical care of individuals with developmental disabilities. Maintain documentation in an electronic health record and provide limited telehealth per individual’s request/need. Work collaboratively with a multidisciplinary team to integrate care with specialty providers and other clinicians. Participate in “On-Call” telephone coverage for off hours with team. (Currently divided with 5 other providers)
Additionally:
• Work collaboratively with Nursing, physical/occupational/speech therapists and nutrition services to develop individualized therapy plans as necessary.
• Review discharge plans from hospitalizations to resume care in residences. Educate staff on hospital discharge plans.
• Prescribe medications and other treatments to treat medical issues.
• Maintain medical records in the EHR system and provide justification for billed services.
• Discuss patient condition, treatment, and prognosis with family members and caregivers and answers their questions.
• Work with multidisciplinary treatment teams to help maintain optimal physical functioning for individuals.
• Coordinate and supervise the treatment and care for a caseload of indviduals.
• Develop and implement training programs for staff.
• Participate in Meetings and training as appropriate.
• Other duties as assigned.
