Minimum Qualifications PROMOTIONAL OPPORTUNITIES:
Candidates currently reachable on the Civil Service List for Licensed Master Social Worker 1.
TRANSFER OPPORTUNITIES:
Candidates must have one year permanent competitive or 55-b/c service in this title and be eligible for lateral transfer; OR be eligible for transfer via Section 70.1 of Civil Service law. To qualify for Section 70.1 transfer, candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) and have at least one year of permanent competitive or 55-b/c service in a title approved for transfer by Civil Service within two salary grades of the Grade 20 level.
MINIMUM QUALIFICATIONS:
Candidates must be active and reachable on the appropriate Civil Service eligible list for Licensed Master Social Worker 1
And:
Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience.
OR B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).
PROVISIONAL APPOINTMENT:
Must be licensed and currently registered by New York State as a Licensed Master Social Worker.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Perform Front Door assessment/EAA tool for individuals who are transitioning out school into adult services with OPWDD
• work with district personnel in identifying upcoming graduates, scheduling Front Door sessions, and assisting in transition planning to adult services
• track and report data on each graduating class
• work with families in the transition planning process and document communications
• maintain dialog with families, both in writing and by telephone regarding their child and the placement process
• attend meetings with school district personnel, voluntary providers, advocacy organizations and other state agencies as required.
• Update all trackers and databases timely
• Other assignments as specified by the Regional Office Leadership
Selection Criteria:
• Possess excellent and professional communication, public speaking, writing and organizational skills.
• Ability to initiate and maintain working relationships with people served as families, advocates, community representatives, voluntary providers, DDRO, DDSOO and other agency staff.
• Excellent computer knowledge and ability to maintain databases and tracking systems.
• Ability to work independently and as part of a team as required as well as provide direct supervision to unit staff.
• Possess acceptable time and attendance work record.
• Ability to participate in requested functions and activities during evening hours and weekends as requested.
