Minimum Qualifications PROMOTIONAL OPPORTUNITIES:

Candidates currently reachable on the Civil Service List for Licensed Master Social Worker 1.

TRANSFER OPPORTUNITIES:

Candidates must have one year permanent competitive or 55-b/c service in this title and be eligible for lateral transfer; OR be eligible for transfer via Section 70.1 of Civil Service law. To qualify for Section 70.1 transfer, candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) and have at least one year of permanent competitive or 55-b/c service in a title approved for transfer by Civil Service within two salary grades of the Grade 20 level.

MINIMUM QUALIFICATIONS:

Candidates must be active and reachable on the appropriate Civil Service eligible list for Licensed Master Social Worker 1
And:

Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience.

OR B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).

PROVISIONAL APPOINTMENT:

Must be licensed and currently registered by New York State as a Licensed Master Social Worker.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Receive Information Line calls (to Albany and local DDRO) that are directed to RO5 and require intervention by a care coordination organization (CCO) care manager,
• Request follow up from care managers and enter resolution on customer service log,
• Receive and investigate calls from families that are experiencing concerns with care management services and add concerns to customer service log,
• Receive and investigate concerns submitted by care managers about provider agencies,
• Organize and host monthly Care Coordination Organization (CCO) meetings with three CCO's covering Long Island,
• Organize and host quarterly care manager and care manager supervisor meetings at the LIDDRO,
• Attend care manager quarterly meetings hosted by OPWDD Central Office (CO),
• Attend Care Coordination Support Liaison (CCSL) Community of Practice meetings hosted by OPWDD CO,
• Process CCO2 forms for disenrollment of individuals from care management agencies,
• Verify lost contact cases submitted by CCO's for disenrollment,
• Process requests for CCO overrides submitted by care managers,
• Troubleshoot delays for CCO enrollment,
• Provide information and support to care managers about services and program vacancies,
• Provide assistance to care managers about special cases that require additional regional office or CO intervention,
• Track concerns/trends of consistent care manager issues, such as difficulty in completing Life Plans,
• Maintain monthly statistics of calls processed, overrides completed and CCO1's that need troubleshooting,
• Participate in special projects, as requested.
