Minimum Qualifications PROMOTIONAL OPPORTUNITIES:

Candidates currently reachable on the Civil Service List for Licensed Master Social Worker 1.

TRANSFER OPPORTUNITIES:

Candidates must have one year permanent competitive or 55-b/c service in this title and be eligible for lateral transfer; OR be eligible for transfer via Section 70.1 of Civil Service law. To qualify for Section 70.1 transfer, candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) and have at least one year of permanent competitive or 55-b/c service in a title approved for transfer by Civil Service within two salary grades of the Grade 20 level.

MINIMUM QUALIFICATIONS:

Candidates must be active and reachable on the appropriate Civil Service eligible list for Licensed Master Social Worker 1
And:

Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience.

OR B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).

PROVISIONAL APPOINTMENT:

Must be licensed and currently registered by New York State as a Licensed Master Social Worker.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Self-Direction Liaison performs administrative duties related to the operation of the Self Direction Unit, including assisting with planning, development, implementation, operation and evaluation of Self-Directed Services. Keeping apprised of new initiatives as well as keeping current with changes in existing programs is imperative to the liaison.
Primary Functions
1) Operational Support
A. Oversee Plan Development
• Complete Quality Review for new authorizations through Front Door and Service Amendment.
• Assist Circle of Support with start-up and plan development.
• Manage plan review process.
• Manage plan through Central Office process, providing advocacy when necessary.
• Secure appropriate funding for Self-Direction budget plan.
B. Manage approved Self Direction plans
• Liaison with participant and families, Care Managers, brokers and Fiscal Intermediary agencies to assist.
• Process plan amendments, ensuring quality and completeness.
• Ensure proper safeguards are ongoing.
• Provide support and necessary referrals for individuals who do not have brokers funded through their plan.
• Utilize SharePoint site to send and receive Self Direction budgets with Central Office.
C. Provide Support and Assistance to Self-Directed Participants in Crisis
• Receive information and distribute it to LIDDRO administration and LIDDRO Crisis Unit.
• Provide feedback and support to all stakeholders to ensure safeguards are in place to avoid recurrence
• Assist in timely amendments to budgets as needed
• Follow-up to ensure safeguards were implemented.
2) Provide/Oversight of Consultation Service
• Provide consultations for individuals and stakeholders.
• Facilitate Self-Direction Information Sessions, Self-Direction Broker Training and Broker refresher trainings.
3) Maintain Compliance with Medicaid Administration Memorandums and Documentation
• Review and maintain records compliance
• Review Life Plan, DDP2, IEP, BMP, and other supporting documents to ensure the Self Direction plan is accurate
• Oversee the maintenance of Start Up Brokerage and Support Brokerage agreement documentation system
4) Maintain Tracking and Oversight for Self-Directed Participants with Housing Subsidies
• Adhere to current policy regarding approval for housing subsidies
• Request all required documentation as described in current ADM.
• Ensure stakeholders continue to send all pertinent housing documentation to the DDRO to maintain the housing subsidy.
• Update housing tracker on a monthly basis.
