Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:

• Reachable on the Civil Service List for Social Work Assistant 1 or 2; OR
• Currently serving as a permanent Social Work Assistant 1 or 2 eligible for transfer; OR
• One year of permanent competitive service in a title eligible for transfer under Section 70.1 of the NYS Civil Service Law.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Complete monthly visits to Family Care homes as required by OPWDD guidelines, complete Form 239 ensuring that environmental requirements related to FCP are being met and maintained.
• Ensuring that all program and environmental requirements related to their Family Care homes are being met including the safety of the home and the health and safety of the individuals.
• Assessing the appropriateness of each individual's activities based on the Residential Habilitation (RHP) and documenting that Residential Habilitation Services are being delivered in accordance with the RHP (HCBS) setting rules.
• Processing all RHP payments through TABS as well as any other reimbursements made to the Family Care Providers.
• Reviewing the Personal Allowance Ledger monthly to ensure the appropriateness of expenditures with receipts as required.
• Reviewing and copying all related records including financial and medical records, as necessary.
• Review the Personal Allowance Ledger, Personal Expenditure Plan (PEP) and count cash-on-hand quarterly.
• Assisting Family Care Providers with the recertification process.
• Complying with the policies/procedures for Family Care and HCBS setting rules.
Intervene/mediate when there are unresolved issues between the FCP and the individual or the family member.
• Communicate with the Family Care Coordinator and administrative staff on any issues within the Family Care Program that might compromise the individual’s safety of the home.
• Communicate with the team including the Care Manager, regarding recommendations to address the individuals needs, safeguards and Life Plan.
• Ensure the FCP receives updates for regulations, policies and procedures as well as any other changes made to the Family Care Manual.
• Work with the individual, FCP and the team, including the Care Manager, to develop the Staff Action Plan and Safeguard Summary, consistent with the Life Plan.
• Ensure the FCP receives and understands the current Staff Action Plan.
• Ensure the FCP is able to carry out the Staff Action Plan as identified, and the Safeguard Summary accurately addresses the supervision and support needs of individuals.
• Ensure the FCP is made aware of any changes on payments or benefits on behalf of the individual.
• Ensure the FCP understands how to complete the HCBS waiver as require.
• Working to achieve OPWDD's Mission by maintaining individuals in Family Care and helping them to live richer more independent lives, enjoy meaningful relationships with friends, family and others, and fully participate in their communities.
