Duties Description POSITION DESCRIPTION:
Provide professional nursing services to patients in the Comprehensive Adult Transition Home (CATH)**

** Comprehensive Adult Transition Home (OPWDD)
An 18-month transition unit for Developmentally Disabled Adults with psychiatric disabilities who are transitioning from residential school placement and who require intensive treatment to prepare to live in a community setting.

Additional Comments • Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($61,270), Increased Hiring Salary ($8,416), Downstate Adjustment ($3,026), and Geographic Differential ($35,000). Geographic Differential added to the job rate ($77,912) for PS&T Grade 18.

• As a participating agency in the Health Care and Mental Hygiene Worker Bonus Program (HWB), direct care employees may be eligible to earn up to $3000 in bonuses for qualifying service if they meet specific requirements including continuous service during the appropriate vesting periods. • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
