Duties Description POSITION DESCRIPTION:

As a Habilitation Specialist 1, you would carry out a variety of activities in support of individuals diagnosed with intellectual and/or developmental disabilities; and assist them in developing pre-academic, academic, self-help, communication, prevocational, and appropriate community skills. You would assist in the development and evaluation of individual habilitation plans and setting goals; monitor and record individuals’ progress; provide guidance and training to direct care staff in the implementation of goals and service plans; and may supervise direct care staff.

PRIMARY DUTIES AND RESPONSIBILITIES:

• Development of program plans based on each individual’s valued outcomes.
• Documentation of program-specific plans.
• Participates in individuals’ Life Plan meetings.
• Participates in activities which support the program(s) and/or the unit’s approach to providing optimum care for all individuals.
• Oversee all aspects of special components of the program which occur regularly.
• Verify all program services are provided as prescribed by team planning process.
