Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
• Reachable on the Civil Service List for Habilitation Specialist 2 OR currently permanent in a title eligible for 70.1 transfer per Civil Service Rules and Regulations.
QUALIFYING EXPERIENCE FOR APPOINTMENT FROM THE ELIGIBLE LIST:
After one year of the service described in the Minimum Qualifications, successful candidates will be qualified for appointment from the eligible list.
MINIMUM QUALIFICAITON FOR PROVISIONAL APPOINTMENT:

• In the event that there is not a valid eligible list, candidates must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service as a Habilitation Specialist 1, Habilitation Specialist 1 (Spanish Language), or Habilitation Specialist 1 (Manual Communications).
• Only the title(s) listed is qualifying for this provisional appointment.

Note: If you are appointed provisionally, you must take the next scheduled Civil Service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained. Service as a provisional appointee is at the discretion of the appointing authority.

Duties Description • Support individuals with intellectual and developmental disabilities in developing job readiness and vocational skills
• Maintain day program certification according to regulatory requirements
• Ensure that all documentation is entered in the electronic health record and maintained in accordance with regulation and policy
• Work cooperatively with day program staff, clinical staff and residential teams to support achievement of person centered goals and valued outcomes
• Provide oversight to Habilitation Specialist 1 and program activities
• Provide specialized training, when required, and guidance to staff on a day-to- day basis. Ensure required training of staff is completed as per agency requirements and individual’s needs
• Attend Life Plan meetings and collaborate with individuals, family members, advocates, Care Managers and service providers to ensure coordination of services
• Ensure activities are person centered and meet individuals’ needs
• Ensure the planning of community inclusion activities for each individual and the delivery and documentation of community inclusion services in accord with each individual’s person centered valued outcomes
• Ensure that incidents are reported and documented appropriately, and recommendations for follow up actions are completed in accordance with investigator and/or IRC recommendations
• Complete investigations as assigned for minor notable incidents occurring at LIDDSO service locations
• Applicant must obtain PROMOTE certification if not currently certified
• Applicant must be First Aid and CPR certified
• Additional duties as assigned by supervisor
• Travel may be required
