Minimum Qualifications MINIMUM QUALIFICATIONS:

1. Must currently be a permanent Developmental Disabilities Program Specialist 1 OR

2. Must be active and reachable on the Civil Service eligible list for Developmental Disabilities Program Specialist 1 OR

3. Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to Developmental Disabilities Program Specialist 1 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title.


You must be a qualified employee of the New York State Office for People with Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service in a position at a Grade 16 or above.

NOTE: Provisional appointments will only be considered if there are insufficient in-title and/or Civil Service list candidates.


This position will be a supervisory role assigned to the Regional Office Region 4 Front Door Team. As such will conduct tasks and supervise staff conducting tasks related to individuals and families seeking OPWDD services, monitoring the Care Coordination Organizations and the Children’s Waiver duties.


This is a supervisory position assigned to the Front Door division of the Regional Office with a primary responsibility for oversight of the Care Coordination Support Liaisons. Although as a supervisor in the Front Door this role may extend to other teams in the unit as needed (i.e.: Traditional Front Door and Children’s Waiver).
Supervising a team of Care Coordination Support Liaisons Includes but is not limited to Training, Guidance, and completion of the following:

• Processing CCO2 submissions in CHOICES.

• Vetting and subsequent requesting of CCO overrides from Central Office

• Troubleshooting general CM and individual/family issues including, but not limited to, linking CCOs to appropriate DDRO contacts, navigating CHOICES, communication with stakeholders via phone and email, and sharing community resources/contacts.

• Facilitating local Care Manager training to address local needs.

• Responding to Code 95 removals requests and all pertinent follow-up involved.

• Reviewing, processing approvals for SFAR (NMCM, state-paid services such as respite and CH). Including tracking and follow-up to ensure continuity from year to year.

• Setting agenda and Leading Monthly CCO leadership Meetings
