Minimum Qualifications Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)

OR

Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying.

** Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.

Duties Description • Will carry a caseload and work with individuals, families, Care Managers (CMs), and other stake holders who come in through the Front Door and Crisis Unit and currently in need of immediate services.
• Consults and interfaces with hospitals, APS/CPS, school districts, families, and provider agencies with complex cases to facilitate appropriate options within Family Support Services, Waiver and CSIDD services.
• Gather information regarding known Individuals in Hospital for extended stays (Hospital Tracker Form), and update as discharges occur
• Field work, which may involve home visits, outside meetings with CMs, private agencies, and families, etc., strong computer skills, and ability to prepare accurate and timely written records and reports are a must.
• Independently gather information on the crisis at hand; based on the initial information, utilize CHOICES application to gather initial supporting documentation.
• Provide referrals and recommendations based on specific case needs (mobile crisis, NYSTART, Comhab, Crisis respite)
• Maintain accurate records of communication with all parties involved in cases
• Attend interdisciplinary meetings (at hospitals, schools, day programs, crisis houses) for planning purposes
• Participate in conference calls with hospitals, COS Team Members, Crisis Respite Houses
• Participate in weekly team meeting to discuss crisis cases (concerns, progress)
• Special projects as assigned by Administration.
• Excellent problem-solving skills and knowledge of OPWDD services are required.

Selection Criteria:
• Possess excellent and professional communication, public speaking, writing and organizational skills.
• Ability to initiate and maintain working relationships with individuals served, families, advocates, community representatives, voluntary providers, DDRO and DDSOO agency staff.
• Computer knowledge and ability to maintain databases and monitoring systems.
• Ability to work independently and as part of a team as required, as well as provide direct supervision to unit staff.
• Possess acceptable time and attendance work record.
• Ability to participate in requested functions and activities during evening hours and weekends as requested.
