Minimum Qualifications Candidate must be reachable on the current Civil Service list for Habilitation Specialist 2
OR
Candidate must have one year of permanent competitive or 55 b/55c service as a Habilitation Specialist 2 and eligible for transfer.
OR
At least one-year permanent service in a title eligible for a 70.1 transfer pursuant to Civil Service Law.

Eligible for provisional appointment pending examination. To be eligible for provisional appointment, candidates must have served at least one year in the Habilitation Specialist 1 and all parenthetic.

Duties Description •Candidate has multiple sites and reports to main office, some driving is required.
•The Habilitation Specialist 2 evaluates individuals diagnosed with Intellectual Disability to generate effective habilitative program plans concurrent with their needs.
•The person in this role is a coordinator of habilitation services, health services and financial services.
•Works with team to secure entitlements and benefits remain active.
•Through the team process and with feedback from service recipients they develop Staff Action Plans, Habilitative Program plans, generate Safeguard Summaries and work for program participants in reaching their optimum level of functioning.
•They work with other stake holders to schedule Life Plan meetings as regulated and adhere the Medicaid standards for services.
•Generate records electronically and follows organizational structure of electronic submission.
•Keeps up to date records for agency and survey purposes including surveys as necessary.
•Review records for training needs of direct support staff
•File records as per retention policy
•Train direct support staff on the needed supports and steps to each individual’s Habilitative Program Plan, safety measures for individual and habilitative service delivery into the electronic format.
•The candidate will work within an interdisciplinary team and is responsible to generate monthly notes on the program planning outcomes, amend supports for individuals to reach to their potential.
