Requirement for prescribing medication: You must meet the current requirements as prescribed by the New York State Board of Nursing.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The Nurse Practitioner – Family Health provides primary care services to assigned residents of group homes.
This includes mandated physicals and coordination of all acute and chronic care including coordination of multiple
specialists for our complex patients. The Nurse Practitioner develops expertise in the care of developmentally disabled
and psychiatrically ill residents consistent with our goal of being the safety net provider.
The function of primary care to our residents is accomplished with a team of Nurse Practitioners and Physicians in the
Medical Dept in combination with Nursing and Direct Care Staff. The nurse practitioners are included in Practitioner On
Call duty to provide 24/7 availability to address any medical problem that may develop. This requires fully licensed [NYS
and DEA] practitioners.
The primary job performance requirements are below:
• Provides professional medical services to individuals in a manner reflecting the stated values of OPWDD:
compassion, dignity, diversity, excellence, and honesty.
• Ensures licensure and registration to practice remain current at all times.
• Attends continuing medical education programs to keep up to date with current trends in the practice of medicine.
• Each Part Time NP will participate in the “Practitioner On Call” (POC) rotation.

Additional Comments STARTING SALARY: $151,897* this salary will be pro-rated to 50%.
OTHER: *Please note, the posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 24 ($88,639), along with any special salary treatments in effect for this position in this location, such as a Geographic Differential ($40,000) and increased Hiring Rate ($23,258)).
