Duties Description The incumbent will serve as the Chairperson of the Incident Review Committee (IRC) and act as the primary liaison with State Operations to ensure the prompt and thorough submission and investigation of all incidents involving the DDSOO. Additionally, the incumbent will handle notifications and provide training and technical support related to the incident review process as needed for the facility.
Duties and responsibilities include but are not limited to:
• Coordinating and updating training programs in compliance with state regulations.
• Contributing to the development and implementation of new employee orientation programs.
• Performing accurate data entry of incidents into the Incident Report and Management Application (IRMA).
• Monitoring agency compliance with incident management regulations and internal policies.
• Maintaining knowledge of Willowbrook reporting and documentation requirements as they relate to incident handling.
• Ensuring strict adherence to incident reporting procedures and timelines.
• Reviewing and tracking internal (In House) incidents comprehensively.
• Leading bi-weekly IRC meetings and ensuring timely documentation and entry of minutes into IRMA.
• Conducting bi-weekly incident tracking meetings with Team Leaders and/or Managers and distributing updated minutes promptly to all relevant stakeholders.
• Managing the Incident Review Hotline, documenting calls, and ensuring appropriate follow-up actions are taken and recorded.
• Preparing and mailing all required OPWDD form 148s and other necessary documentation punctually.
• Tracking and reporting on RIA data within the district.
• Processing Jonathan’s Law requests promptly according to regulatory standards.
• Compiling the annual trend report for the IRAs/ICF and presenting findings to the IRC and Quality Council (QC).
• Conducting additional trend analyses as requested and providing reports to the Director of Quality Management and the DDPS-2 of QM.
• Serving as a liaison with residences, IMU, OIIA, the Justice Center, and other relevant departments regarding incident investigations and status updates.
• Cross-training with the Quality Management Coordinator to provide backup support.
• Participating in QM/IR department meetings to stay current on tasks and assist with follow-up assignments as directed by the QM/IR Director or designee.
• Cross-training on any other department functions as directed by the QM/IR Director.
• Performing any additional duties assigned by the Director of Quality Management and/or their designee related to the Quality Management Department.

Minimum Qualifications Must currently be a permanent Developmental Disabilities Program Specialist 1 and interested in transfer.
OR
Must be active and reachable on the Civil Service eligible list for Developmental Disabilities Program Specialist 1.
OR
Transfer: Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments • Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for Grade 21 ($78,126) and Downstate adjustment ($4,000). Downstate adjustment is added to the job rate ($99,056).

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
