Duties Description The goal of the Care Coordination Organization Health Home (CCO/HH) Program is to improve care and health outcomes, lower Medicaid costs, reduce preventable hospitalizations/emergency room visits and ensure appropriate coordination of care for individuals with I/DD. There are currently more than 123,000 individuals enrolled in this program.

Since the implementation of this program in July 2018, continued monitoring, oversight and coordination assistance has been essential to the success of the program. This position is on the front lines of that coordination - responsible for the detailed operations and communication among various partners that are crucial to its operational success.

Responsibilities of the position include but are not limited to:

· Provide review of upcoming program and policy changes with goal of identifying impact on the CCO/HH program.

· Identify/oversee training development for CCO Care Mangers, OPWDD Regional Office staff and provider agencies related to administrative memorandums, policy requirements and program updates and initiatives as it impacts the CCO/HH program.

· Serve as a Bureau representative on statewide program liaison calls (e.g., Self-Direction, Front Door, Waiver, Children's liaison, etc.)
· Coordinate meetings of OPWDD Developmental Disabilities Regional Office (DDRO) Care Coordination Support Liaisons (CCSLs) and Care Managers (CMs) with the goal of sharing current CCO policy and program information. (Including planning and leading the Quarterly Care Manager's Conferences).

· Develop Care Coordination E-Visories and process documents to communicate important subject matter, including process flows to CMs and others related to Care Management.

· Track and maintain records for training, communications with and notifications for CCO program operations.

· Monitor, review and respond to questions received in CCO Bureau's shared mailboxes.

· Serves a liaison for people receiving services and their families to share their experiences via quarterly care managers trainings.

· Other duties as assigned.

Minimum Qualifications Promotion:
Candidates must be reachable for appointment on the current Civil Service Eligible List.

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
