Minimum Qualifications Candidates must be employed by New York State to be eligible to apply for this position.
Candidates must be permanent in the Mental Hygiene Nursing Program Coordinator title and be eligible for lateral transfer; OR candidates must be eligible to transfer via Section 52.6 or 70.1 of the Civil Service Law by having at least one year of permanent service in a qualifying title within one M grade (or two salary grades) of the M-1 (Grade 25) level.
In the event there is no viable Civil Service list, a provisional appointment may be made. In order to be eligible for a provisional appointment, candidates must have taken the recent Civil Service examination held for this title on September 9, 2017. Their score must be reachable on the resulting eligible list before they can be permanently appointed. Candidates who do not pass the examination or achieve a reachable score may not be retained.
Duties Description The Office of Mental Health (OMH) is seeking a qualified candidate to fill a Mental Hygiene Nursing Program Coordinator, M-1, position. Under the general direction of the Mental Health Program Manager 1 (Chief Executive Nursing Officer), the Mental Hygiene Nursing Program Coordinator will serve as the Central Office lead responsible for the coordination of all activities for the Nursing Home Enhanced Support Program as assigned and the implementation of evidenced-based nursing practices. The incumbent will work with Chief Nursing Officers, Community Mental Health Nurse (CMHN) and members of the Treatment Team, the Discharge Planning Team, the Psychiatric Center (PC) psychiatrists and staff, Skilled Nursing Facility (SNF) staff, Patient Resource Offices and Field Office staff on the collaboration, coordination and provision of enhanced supports to individuals discharged to skilled nursing facilities. Specific duties to be performed may include, but are not limited to, the following:
Identify and collaborate with all stakeholders in problem solving and shared-decision making to address systems barriers and regional issues impacting the successful transition and sustainment of individuals in nursing homes.
Collaborate with the PC, SNF and CMHN responsible for the submission of the monthly reports, data and the reporting on the status of individual patients at the designated intervals post discharge from the PC and admission to the SNF.
Arrange and participate in weekly project planning and implementation meetings at each assigned facility.
Oversee the agreement between the OMH PC and SNF to provide convalescent care and ensures OMH provides appropriate access to specialized psychiatric consultation services.
Develop, provide, and/or arrange for in-service education and training for PC nursing staff on best practices and interventions to better prepare long term individuals for placement in skilled nursing facilities.
Collect and compile individual SNF tracking and status data for submission to the OMH Office of Planning within the required timeframes.
Participate in committee meetings and perform other duties as assigned.
