Minimum Qualifications MINIMUM QUALIFICATIONS:
Permanent Qualifications: Candidates must currently be a permanent Social Work Assistant 3 OR be reachable on the current Civil Service eligible list
Transfer Qualifications: Candidates must be eligible for transfer under Section 70.1 of the Civil Service Law.
Open Competitive Qualifications: Candidates must have a master’s degree in social work OR a bachelor’s degree in social work and ONE year of post-bachelor’s experience in social work OR a bachelor’s degree in a human services area and TWO years post-bachelors experience in Social Work.
**QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a Bachelor's Degree or higher in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling or psychology).
Duties Description POSITION DESCRIPTION:
The Social Worker Assistant 3 (SWA3) will provide supervision and oversight to the Family Care Program in Ulster and Dutchess counties, while covering a caseload of 10-15 individuals in Family Care Homes. The SWA3 is responsible for ensuring that the safety and health of the individuals are maintained in the Family Care Homes. They will also be responsible for monitoring the homes to ensure that any health and safety issues are immediately addressed and that the required follow up is completed.
PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include but are not limited to:
? Supervisor of Family Care Home Liaison in Ulster and Dutchess counties
? Medicaid compliance review of Family Care Medicaid files in office and Family Care home
? Ensure communication occurs between the Family Care Providers and the other members of the treatment team
? Monitor Family Care Homes to ensure living environments meet the individuals physical, medical, social and spiritual needs
? Acts as a liaison between the Family Care Provider, the individual and the other treatment team members
? Ensure that the Family Care Providers adhere to family care policies relative to the health and safety of the individuals in their Family Care Home
? Monitor and update residential habilitation plans as well as other regulatory paperwork and ensure that the Family Care Providers are following regulatory requirements.
? Complete investigations on an as needed basis
? Ensure that Family Care Providers are adhering to the individual plans of care

This may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.
