Minimum Qualifications Candidates must be a current NYS employee and have: a Bachelor’s Degree in Social Work; OR a Bachelor’s Degree in a Human services field plus ONE year post Baccalaureate experience in Social Work; OR a Master’s Degree in Social Work; OR two years of permanent competitive or 55-b/c experience as a Social Work Assistant I; OR permanent or contingent/permanent status in a grade 12, 13, or 14 direct care position and eligible for a 70.1 transfer; OR permanent or contingent/permanent status in a Social Work Assistant 2 position OR reachable for appointment from the Civil Service List; OR eligible for a 70.1 transfer.

Duties Description This position is essential to the core mission of OPWDD transformation agenda. This position is for the Front Door/Eligibility unit for Metro Bronx DDRO.

• As a Social Work Assistant 2 in the Front Door/Eligibility
Department, the candidate will do follow up for the Front Door
facilitators; ensuring that open EAAs are completed. Contact
with families, follow up on waiver enrollment documents, close
out EAAs for those who are no longer interested or with changed
status.

• Assist with Front Door information sessions.

• Assist families with getting needed information to enroll them
into waiver.

• Assist with Intake of new individuals both in person and over the
phone.

• Open TABS for individuals coming through the Front Door.
