Minimum Qualifications Provisional Qualifications:
Open Competitive candidates must have a master’s degree in social work OR a bachelor’s degree in social work and ONE year of post-bachelor’s experience in social work OR a bachelor’s degree in a human services area and TWO years post-bachelors experience in Social Work.

Duties Description The Social Work Assistant 3 (SWA3) position will serve as an integral staff member of the Front Door team at the Taconic Regional Office. The Front Door is the gateway to OPWDD services and is critical to the Core Mission of the Agency. The SWA3 will assist with individual’s eligibility for services and connection to services, therefore this position directly impacts the Health and Safety of the individuals served by OPWDD.

PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include but are not limited to:
• Act as a liaison between the family, MSC and the Regional Office
• Assist with Eligibility, Assessment and Authorization for each person
• Assist individuals with developmental disabilities and their families to connect with authorized services.
• Participate in the Quality Review committee to ensure the person gets the most appropriate services
• Act as the Residential School Transition Front Door liaison
• Participate in the monthly Front Door information sessions
• Assist in Medicaid parental deeming for children under 18 and waiver enrollment

Additional Comments : You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. This may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.
