Minimum Qualifications Transfer Qualifications: Any current Habilitation Specialist 1
Any permanent employee eligible for a 70.1 transfer.
Anyone eligible to take the Civil Service examination for Habilitation Specialist 1

Minimum Qualifications: A qualifying Bachelor’s Degree in Human Services and 1 year clinical/or direct care experience or a qualifying Master’s Degree in Human Services.
QIDP standards preferred.
QIDP STANDARDS ARE:
• The Department of Developmental Services (DDS) adopted the federal definition for Qualified Intellectual Disabilities Professional (QIDP) in all ICF/DD, ICF/DD-H, ICF/DD-N and DD-CNC facilities when the Federal ICF/IID regulations became effective in October 1988. The federal criteria out dates the state regulations specified in the California Code of Regulations, Title 22.
• QIDP is a staff who is responsible for integrating, coordinating and monitoring each client's active treatment and who possesses:
• (a) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
• (b) Is either a physician and surgeon, or an osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• Provide assessment, training, and assistance to 14 individuals to develop skills promoting independence in classroom and alternative settings.
• Monitor programs and revise as needed to ensure continual progress of individuals in reaching their goals.
• Review and update objectives for individuals at specified times and when necessary to meet the needs of the individual.
• Instruct paraprofessionals in appropriate strategies to be used with individuals.
• May supervise activities of paraprofessionals who assist in delivering programs to individuals.
• Review performance data and program plan at specified intervals to assure individuals continual progress.
• Ensure that individuals participate in community inclusion activities by transporting and assisting individuals and documenting these activities.
• Assess current functioning of individuals in relation to valued outcomes. Communicate with treatment team members to gather additional information to plan appropriate services. Make modifications to services as recommended by the treatment team.
• Ensure individuals are provided with the appropriate level of supervision, based upon their needs.
• Record data and monthly summaries in a timely manner and in accordance with agency standards, and ensure they are complete and up-to-date.
• Will function as the classroom leader and provide training/guidance on the individuals’ program goals.
• Develop and implement lesson plans for Social Skills, Anger Management, and Health and Wellness.
• Liaison for the individuals while on community inclusion activities.
• Work as a member of an interdisciplinary treatment team.
• Attend and participate in all mandatory in-service training and staff meetings.
• May be asked to assist with other habilitation service needs as warranted.
• Participate in chart audits.
• Assist with maintenance and development of the daily program schedule, development of day program schedule and coordination.
• Assist in managing Rewards Store at OFS.
• Documentation, follow up and care of adaptive equipment.
• Provide assessments and attend individuals special, quarterly and annual meetings.
• Other duties as directed/assigned.
