Minimum Qualifications MINIMUM QUALIFICATIONS:
• Must currently be permanent in the title of Licensed Master Social Worker 1, OR
• Must be active and reachable on the appropriate Civil Service eligible list, OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by
the NYS Department of Civil Service.

FOR PROVISIONAL APPOINTMENT:
• Must possess a Master’s degree in Social Work from a regionally accredited college or university, OR
A Bachelor’s degree in Social Work and one year of post bachelor’s experience in Social Work, OR
A Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.
If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained *

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
1. Interface with hospitals who have OPWDD individual in their care
2. Write comprehensive psycho-social summaries when necessary
3. Make appropriate residential referrals based on the individual’s needs
4. Track Certified Residential Opportunities referrals and follow-up with agencies in a timely fashion
5. Assess cases and present during monthly meetings
6. Interface with agencies providing residential services
7. Enter information into the Residential Opportunities database
8. Additional responsibilities to be determined based on unit needs

Additional Comments Please note:
• If you are employed by state government you may be able to receive loan forgiveness
under the Public Service Loan Forgiveness Program (PSLF). The PSLF Program forgives
the remaining balance on Direct Loans after 120 qualifying monthly payments have been
made under a qualifying repayment plan while working full-time for a qualifying employer.
