PROVISIONAL QUALIFICATIONS:

1. You must have your current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD). You must indicate your RD registration number on your application.

OR

2. You must have your New York State Education Department (NYSED) certification in dietetics-nutrition AND have completed a minimum of an additional 400 hours* of supervised dietetics practice under the supervision of a RD. You must indicate your NYSED certification in dietetics-nutrition number on your application.

*If you are qualifying under #2, the additional 400 hours required is in addition to the 800 hours of supervised dietetics practice under the supervision of an RD required for NYSED certification. On your application, you must provide the name of the institution where you obtained the additional hours; the name of your supervisor; and your supervisor's qualifications.

Candidates must maintain current certification in New York State as a Certified Dietitian/Nutritionist AND maintain current registration with Commission on Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics. Certified Diabetes Educator (CDE) credentialed dietitian highly sought. Academy membership is highly recommended.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Coordinate medical records for clinical nutrition. Ensure medical records reflect nutritional status of the individuals and meet regulations.
• Maintain annual completion of assessments using Nutrition Care Process.
• Review menus for regular and modified diets.
• Attend ISP meetings and discharge meetings.
• Complete nutritional assessments, care plans and discharge summaries.
• Make recommendations to the treatment team for the maintenance and/or improvement of nutritional status.
• Recommend changes to meal plans to address nutritional concerns.
• Work as part of an interdisciplinary team to assess feeding and swallowing concerns.
• Schedule and conduct Nutrition in-service education/training for house staff. Maintain attendance records and record topics covered.
• Implement plans for corrective action.
• Ensure OPWDD regulations are adhered to and for overall nutritional health and safety of the individuals.
• Regular and proficient computer use.
• Conduct regular IRA sanitation inspections on assigned community homes.

Frequent travel between assigned IRAs in Oneida and Lewis Counties is expected. Mileage is compensated per Office of State Comptroller rules and regulations.

Additional Comments Important note: the candidate selected to this position is advised that it will be their responsibility to discuss any previously approved time off with their new supervisor. There is no guarantee the new unit will be able to honor previously approved time.
