Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
Candidates must be reachable on the current Civil Service List for Community Mental Health Nurse
OR currently permanent in a title eligible for 70.1 transfer per Civil Service Rules & Regulations.

MINIMUM QUALIFICATIONS FOR TEMPORARY APPOINTMENT:
Candidates must be licensed and currently registered to practice nursing in New York State or possess a limited permit to practice nursing in New York State. (You must obtain your New York State license within one year of appointment or action will be taken to remove you from the position).
AND EITHER
1. A bachelor's or higher degree in one of the areas listed below* AND two years of professional clinical nursing experience, including at least one year as described below**
OR
2. 30 college credits in the areas listed below* IN ADDITION TO an Associate’s degree in nursing or completion of a three-year nursing program AND three years of professional clinical nursing experience, including at least one year as described below**
*Acceptable areas including nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages etc. are NOT acceptable.
**One year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a COMMUNITY SETTING, e.g. a halfway house, visiting nurse, or a community clinic. SUBSTITUTION: A master's degree in community mental health may be substituted for this experience only.

Duties Description The candidate will work as a nursing supervisor in Brooklyn DDSO and be responsible for 2 intermediate care facility (ICF ) and may function as a residential nurses as needed.
• Will ensure nursing case management of 2 ICF who have caseloads of 44-50 individuals
• Will over see and supervise RNs and LPNs who will provide supervision to the AMAP staff in their role in providing Direct Support Assistants health care per the nurse Act Exemption.
• Will ensure all medical follow-up is done timely
• Will be responsible for in-service on nursing plans and other staff development and oversight as required, not limited to: medication administration which are vital activities to regular operations.
• Engage in orientation and mentoring of new RNs and LPNs
• Complete evaluations
• Complete nursing audit at the ICF, work closely with NA / NPC and Team to review findings, develop plan of correction
• Monitors individual’s medical/health needs and services.
• Interacts with primary, medical specialists, various clinical staff as necessary.
• Advocate for individuals’ health care needs
• Maintains ongoing communication with med liaison/designee
• Provides support to residential staff in maintaining health services for the individuals.
• Utilizes appropriate reporting mechanisms/chain of command in responding to individual and/or staff issues
• Responds to emergency requests as soon as possible (within and outside caseload).
• Completes nursing assessments and related documentation e.g. RN Notes, nursing log book, etc. thoroughly and within established time frames.
• Provides individual-specific nursing interventions.
• Responds to staff questions regarding individual specific medical/nursing issues.
• Q+As and PONS are current (for caseload) and signed by staff. All PONS are maintained in computer files.
• Provides health related training/consultation to residential staff.
• Annually for everyone in your caseload, completes: review of medical section ISP, medication audit (each home/day program)
Obtain individual’s informed consent for medications, as per BDDSOO Policy.
• Completes required medication error documentation and staff medication error remediation
• Maintains professional skills
• Attends Nursing Discipline meetings.
• Attends/completes on-line mandated DDSO training, as well as available discipline specific training.
• Identifies problems and/or opportunities for personal and/or nursing department improvement (s) and contributes to the development of an action plan.
• Completes special projects/assignments

Starting salary for this title reflects the Increased Hiring Salary, Downstate Adjustment, and Geographic Pay Differential. $79,219-$91,682 Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 19 ($59,671), Increased Hiring Salary ($3,651), Downstate Adjustment ($3,026), and Geographic Pay Differential ($12,871). Downstate Adjustment and Geographic Pay Differential added to the job rate ($75,785) for PS&T Grade 19.

• This position is pending Division of Budget Approval.
