Minimum Qualifications 1. Must currently be a permanent Developmental Disabilities Program Specialist 1.

OR

2. Must be active and reachable on the Civil Service eligible list for Developmental Disabilities Program Specialist 1.

OR

3. Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to Developmental Disabilities Program Specialist 1 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title);

OR

4. Qualified for Provisional Appointment.

Minimum Qualifications for Provisional Appointment:
You must be a qualified employee of the New York State Office for People with Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service in a position at a Grade 16 or above.

NOTE: Provisional appointments will only be considered if there are insufficient in-title and/or Civil Service list candidates.

Duties Description Provide Front Door Lead Services as follows:

Local School Transition (Aging Out) Coordinator:
• Gather demographic date on transitions students
• Compile data needed to complete annual reports on students transitioning from local schools
• Report summary data annually for students transitioning from local schools
• Distribute information on OPWDD and the Front Door to transition coordinators and administrators at local schools for distribution to parents/guardians
• Answer questions from schools, parents and students about transition from local school to OPWDD adult services
• Attend CSE (Committee on Special Education) meetings as needed
• Attend school open houses and transition group meetings when asked and when available
• Refer the student and family to the Front Door
• Work with/assist/support Front door staff who are helping local students go through the Front Door process


The Local School Transition (LST) Coordinator may, on an as assigned or as needed basis:
• Complete EAA Tools for transitioning students
• Meet with school transition coordinators and/or transition coordinator groups/organizations
• Attends transition fairs and conferences in their district
Supervise/provide administrative oversight of Family Support Services (FSS) Coordinators for Westchester as well as Rockland/Orange/Sullivan via the monitoring of the FSS Coordinators’ roles:
• Ensure that each FSS Advisory Council has access to the DDRO Director as needed to carry out its responsibilities;
• Disseminate information to families and individuals including information regarding conferences, trainings, forums and meetings, etc.;
• Provide logistical support, information and technical assistance to the local FSS Advisory Council;
• Advise and train the FSS Advisory Council regarding issues of privacy, confidentiality, and limits to information sharing;
• Provide training to local FSS Advisory Council members in areas requested by the Council;
• Serve as liaison between the FSS Advisory Council, the DDRO Director and Central Office staff, and
• Coordinate the Request for Funding Process for securing local FSS contracts, Advisory Council site visits to program locations, and annual program reviews, inviting local Advisory Council members to participate.

Assist individuals graduating from residential schools and seeking OPWDD supports and services. This will be managed through attendance at meetings as well as processing prerequisite OPWDD eligibility determinations. This process will require the DDPS1 to assist local public and private agencies in the development of new residential programs to meet the residential needs of this group of students.

Access required documentation on OPWDD service recipients, who have expired, for processing in the OPWDD TABS data base. Provides technical assistance by:
• Communicating with Service Providers
• Verifying required data
• Processing information about the individual who had been accessing OPWDD services

HV DDRO TABS/Choices Coordinator
• Provides technical assistance to a variety of groups regarding Choices and TABS enrollment, processing of individual demographic information, enrolling into services, billing for services rendered, etc.
• Other tasks as assigned appropriate to the title.
