Duties Description POSITION DESCRIPTION/PRIMARY JOB DUTIES:

Under the supervision of the Region 2 DDPS I Certified Residential Opportunities (CRO) Team Leader, the incumbent will perform the role of Certified Residential Opportunities Team-Community Specialist for the six counties of the Sunmount region. The primary function of this role is to assist people with developmental disabilities in finding a home of their choice in a community-based setting.
• Work collaboratively with people with developmental disabilities, their families and advocates, and service providers to manage all aspects of a residential referral.
• Obtain, review and distribute referral information for those who have been referred for a residential opportunity.
• Ensure regulatory and procedural protocols related to residential services and movement are followed that ensure the person’s rights, informed choice and safety.
• Assist in the preparation for a person’s move, such as arranging trial visits.
• Assist residential providers with identifying people who may benefit from particular residential opportunities and provide information to Care Managers regarding residential opportunities in the community.
• Maintain the Residential Opportunities report and other informational databases and tracking systems related to Certified Residential Opportunities (CRO).
• Provide training and technical guidance to Care Managers and residential providers about the CRO process.
• Provide as needed guidance and training to individuals and families regarding OPWDD services and supports.
• Assist with preparation and facilitation of weekly CRO meetings and the monthly Access to Residential Opportunities Committee (AROC) meetings.
• Regular travel within the six-county district, and on occasion outside the district for trainings meetings and other assignments.
• The position will also be subject to additional duties related to the Managed Care Transition.
• Other duties as assigned.
