Minimum Qualifications Candidates must be in title OR on the Civil Service eligible list OR be eligible for transfer under Section 70.1 in a designated title determined by the NYS Department of Civil Service. Provisional appointment pending exam may be considered. To be eligible for provisional appointment candidates must be a qualified employee of the New York State Office for People with Developmental Disabilities and have had one year of permanent competitive or 55-b/55-c service in a position at a Grade 16 or above.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The incumbent in this position will be a Self-Direction Liaison for the Regional Office 2, Broome District. The Broome District is comprised of Broome, Chenango, Delaware, Otsego, Tioga, and Tompkins Counties. The role and function of the Self Direction (SD) Liaison is to promote individualized initiatives through self-hired and agency supported Self Direction service options. Familiarity with ISS (Individualized Supports and Services), FSS (Family Supports and Services), CFCO (Community First Choice Option (CFCO) and HCBS (Home and Community Based Services) Waiver services will be required. Travel will be required. The incumbent in this position will provide training throughout the six counties of the Broome District. This position will be subject to additional duties related to the transition to Managed Care. Preference will be given to candidates proficient in Microsoft Word, Excel and PowerPoint; the incumbent in the position will be required to become proficient in the use of these applications. The SD Liaison will perform but not be limited to the following duties and responsibilities:
• Coordination and oversight of Self-Directed plans/services throughout the Broome District of Region 2
• Function as part of the Front Door team for intake/approval of new referrals
• Utilize the Quality Review Team (QRT) process for DDRO approval of service authorizations, service amendments, changes to budgets, etc.
• Participates as a QRT member to review and approve service authorizations and amendments. As a member of the QRT team, the incumbent will need to have a strong understanding of OPWDD services and funding streams (HCBS Waiver, CFCO, State Paid services, etc.).
• Review new and amended budgets; submit to Rate Setting for approval
• Issue price approval letters upon approval/termination of SD budgets
• Provide trainings for Brokers/Care Managers (CMs)/provider agencies, in both small and large group settings. Incumbent will need strong public speaking skills
• Provide training on the Front Door process and Self-Direction for individuals, families, school districts, etc.
• Provide guidance to individuals, families, Brokers and CMs regarding OPWDD service options
• Teach individuals, families, and Brokers how to form and maintain planning teams/circles of support and how to use Person Centered Planning
• Work with CMs, Brokers and Fiscal Intermediary providers to ensure program quality and Medicaid compliance
• Promote understanding of how provision of Self-Direction supports the goals of the OPWDD Transformation Agenda
• Assess and facilitate solutions to address issues and maintain/expand the use of individualized services and budgets
• Maintain ongoing contact with Region 2 Self-Direction liaisons and SD Statewide Coordinators
• Maintain data relevant to Broome District Self-Direction program; report to the DDRO Director and Statewide Coordinators on program status
• Incumbent will perform additional duties related to the transition to Managed Care
• Attend trainings and workshops related to the Self-Direction and related topics
• Ongoing travel within the district is required. Occasional overnight travel is required
• Other duties as assigned
