Salary RangeFrom $56594 to $71218 Annually
Minimum Qualifications • Must currently be permanent in the title of Social Work Assistant 3, OR
• Must be active and reachable on the appropriate Civil Service eligible list, OR
• Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service.
FOR PROVISIONAL APPOINTMENT:
• Must possess a Master’s degree in Social Work from a regionally accredited college or university, OR A Bachelor’s degree in Social Work and one year of post bachelor’s experience in Social Work, OR A Bachelor’s degree or higher in a human service area and two years of post-bachelor’s experience in Social Work.
If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained*
Duties Description The incumbent’s job duties will include, but not limited to the following:
• Assist with cases as assigned
• Ensure that individuals aging out of residential schools receive appropriate OPWDD adult services
• Assist with data management and reporting for individuals transitioning from residential schools to OPWDD adult services, using Excel and Citrix
• Participate in weekly and monthly meetings, both in person, by phone or by video conference with agencies, families, residential schools and developmental staff, all meetings relate to the transition of individuals from residential settings to OPWDD adult services
• Work closely with the Eligibility Team to ensure those in residential schools meet OPWDD criteria to receive services
• Follow the Front Door process including preparing and updating the DDPS2 as necessary
• Visit Residential Schools both in state and out of state for upcoming graduates as needed
• Maintain dialog with families, both in writing and over the phone regarding their child and the placement process
• Review Preliminary Adult Services Plan/Behavioral Plan to ensure the needs of the individual will be met by the OPWDD Adult Residential Opportunity. If the plan is not approved, work with the agency to make corrections, deletions or additions
• Send referrals for residential placement and Day Services to appropriate agencies
• Will cover assigned cases within NYC Region 4
