Minimum Qualifications **Permanent/Provisional**

Permanent Qualifications: Candidates must have QIDP* certification/status and permanent service as a Habilitation Specialist 2 OR be active and reachable on the current Civil Service eligible list for Habilitation Specialist 2.

Transfer Qualifications: Candidates must be eligible for transfer under Section 70.1 of the Civil Service Law.

Provisional Qualifications: Candidates must be scheduled to take the Habilitation Specialist 2 exam, which was announced 9/6/19 and is scheduled to be held on 12/14/19.


PREFERRED QUALIFICATIONS:
* QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a Bachelor's Degree or higher in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling or psychology).

Duties Description POSITION DESCRIPTION:
The Habilitation Specialist 2 (HS 2) at Ulster Day program is a critical supervisory position that oversees the daily operation of the site, providing services for 45 individuals.

PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include, but are not limited to:
? Responsible for the oversight of the daily operations of Ulster Day Habilitation
? Responsible for the coordination, scheduling and facilitating all 6 months reviews
? Develops individual habilitation plans, sets goals, and monitors the individuals progress in reaching these goals
? Monitors daily logs, paperwork and inclusion programs for the individuals
? Ensures completion of monthly notes and sends monthly report to Treatment Team Leader
? Responsible for the physical aspects and compliance issues of the Day Hab
? Coordinates daily transportation
? Assists in programming of individuals, as needed
? Responsible for completion of Medicaid billing, verification and submission on a daily basis

Additional Comments You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. This may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.
