Minimum Qualifications MINIMUM QUALIFICATIONS:
On or before the date of filing your application, you must have one of the following:
1. A license and current registration to practice as a practical nurse in New York State; OR
2. A limited permit to practice as a practical nurse in New York State; OR
3. An application on file for a limited permit to practice as a practical nurse in New York State. (You must be appointed immediately following graduation from an approved school of practical nursing in New York State to qualify under this option.)
Note: If you are appointed with either qualification "2" or "3," you must obtain your license within one year of the date your limited permit was issued or steps will be taken to remove you from the position.

ADDITIONAL QUALIFICATIONS:

• Driver’s License: Valid license to drive in NYS.

• CAPA: Must be cleared by the NYS Central Register for Child Abuse and Maltreatment, if position requires working with minors and not previously cleared.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Administering all injectable, j-tube and g-tube medications to assigned individuals;
• Implementing and monitoring the g-tube and j-tube feedings, including changing g-tubes as necessary;
• Implementing catherization orders, trach care, deep oral and pharyngeal suctioning;
• Any additional medical nursing treatment as necessary;
• Assist with medical needs at IRA’s. Other duties as directed by supervisor or nursing administrative staff. Could be assigned to float to other locations depending on staffing.
• Transport individuals as needed due to medical conditions that require an LPN present
• Other duties as assigned.
