Provisional Appointment: In the event that there is not a valid eligible list, candidates who meet the qualifications below may be qualified for a provisional appointment.
Minimum Qualifications: Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience;**
Or B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).
** Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
• While working in a secure facility provide intensive treatment to developmentally disabled adults who may have been involved with the criminal justice system, will ensure the coordination and delivery of social work.
• Performs a wide variety of social services for assigned individuals.
• Independently provides written social assessments and lends written and verbal input to Treatment team meetings.
• Assesses individuals’ social needs and resources available for optimal social functioning, then develops and implements goal directed programs to meet these needs.
• Monitors individual programs for continuity, consistency and progress as determined by the Interdisciplinary Treatment Team in accordance with ICF regulations.
• Provides one on one therapy, counseling sessions and/or provides classroom instruction to small groups of individuals’ at Program.
• Prepares and conducts in-services for in assessments, programs and other counseling interventions.
• Advocates for the needs of the individuals assigned.
• Assist in referrals to appropriate level of care.
• Provides updated information to Bureau of Revenue and Reimbursement and monitors individual financial situations and acts as individual advocate in determining funding eligibility.
• Obtains informed consent from individuals/families/legal guardians or Informed Consent Committee for any rights, restriction or restrictive intervention.
• Provide administrative supervision to Licensed Master Social Workers 1, and to lower-level paraprofessional staff involved in case management and other treatment and program support tasks that do not required licensure as a licensed master social worker or licensed clinical social worker.
• Teach Social Skills Class as assigned.
• Other duties as assigned.
