PROVISIONAL APPOINTMENT:

Must be a qualified employee of New York State and have had three months of permanent competitive or 55-b/55-c service as follows:

• Habilitation Specialist 1
• Habilitation Specialist 1 (Spanish Language) OR
• Habilitation Specialist 1 (Manual Communications)

If you do not pass the examination or achieve a reachable score, you may not be retained *

Duties Description Under the direct supervision of the Treatment Team Leader, the incumbent will be responsible for, but not limited to:

• Act as the GateKeeper/Supervisor for the Habilitation Specialist 1 to identify and address problems noted in the ISP/Life Plan.
• Coordinates referrals, admissions and discharges of Individuals in cooperation with families, referring agencies, and providers in conjunction with the Individual’s CCO/PHP Care Manager.
• Review Staff Actions for the Unit interact with the CCO and PHP Care Manager and provide feedback to the Habilitation Specialist 1’s.
• Regularly conducts staff meetings to communicate policy and procedure changes, promote team building and a productive work environment.
• Facilitated Electronic Health Record (EHR) trainings for Habilitation Specialist 1.
• Revised Habilitation Specialist 1 caseloads as needed
• Monitors and reviews program plans and charting by Habilitation Specialist 1 to ensure compliance with regulatory and Medicaid standards.
• Maintain a database of Habilitation Specialist 1 caseloads and due dates for the Life Plan.
• Attend Bi-Weekly Peer Review/Supervisory Meetings.
• Travel within the Unit(s)
• Other duties as assigned by Supervisor.
