Minimum Qualifications: Promotional; One year as a Rehabilitation Counselor 1 AND a bachelor's degree in a human service field* or NYS licensure as a Registered Nurse.

Open Competitive: Either 1: a bachelor’s degree in a human service field* AND three years post-bachelor’s qualifying experience;** Or 2: a master’s degree in a human service field* AND two years of post-bachelor’s qualifying experience;** 0r 3: a master’s degree in rehabilitation counseling AND one year of post-bachelor’s qualifying experience.**

** Qualifying Experience: To qualify for the examination you must have professional experience providing vocational, psychosocial, or rehabilitative/habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions. Internship experience does not count if it was credited toward the qualifying degree.

QIDP standards are preferred, as follows:
• The Department of Developmental Services (DDS) adopted the federal definition for Qualified Intellectual Disabilities Professional (QIDP) in all ICF/DD, ICF/DD-H, ICF/DD-N and DD-CNC facilities when the Federal ICF/IID regulations became effective in October 1988. The federal criteria outdates the state regulations specified in the California Code of Regulations, Title 22.
• QIDP is a staff who is responsible for integrating, coordinating and monitoring each client's active treatment and who possesses:
• (a) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
• (b) Is either a physician and surgeon, or a osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The incumbent will:
• Have primary responsibility for oversight of the Long Lake Day Habilitation Program.
• Orchestrate all facets of the active treatment effort.
• Routinely interact with all team members regarding full spectrum of the individuals’ treatment programs.
• Identify inconsistencies in training approaches or programs not being implemented as written and facilitate the resolution of these inconsistencies.
• Assure follow-up occurs for any recommendation for services, equipment or programs so that needed services and supplies are provided in a timely manner to meet the individual’s needs.
• Train team members regarding various components of Individual Treatment Plans (for example, conducting assessments, program goal writing, data collections, etc.), as well as train team members regarding specific individual program plans.
• Work with the Treatment Team Leader and Development Assistant III to assure that issues related to staffing, physical plant and budgeting are impacting positively on the active treatment progress.
• Assist with maintenance and development of the daily program schedule working with the DAIII
• Have some supervisory responsibility over some clinical staff assigned to Long Lake Day Hab.
• Audit program charts Monthly to ensure presence, accuracy, and validity of necessary documents. Follow up as appropriate.
• Provide oversight in the classrooms as necessary.
• Attend ISP meetings.
• Assist Habilitation Specialist in developing individuals program goals.
• Maintain accurate day habilitation billing; train staff as needed on billing requirements.
• Oversee the general maintenance and upkeep of the building and vehicles; report areas of concern to TTL, garage and maintenance department as needed.
• With assistance from the OA 1, manage/audit individuals’ finances necessary for their needs at the day habilitation program.
• Maintain a good relationship with the community.
• Conduct fire drills as outlined in the P&P.
• Coordinate annual employee trainings and all other necessary trainings as needed.
• Other duties as directed/assigned.
