Minimum Qualifications Permanent Transfer Candidates: Current NYS DOH employee with one year or more of permanent or contingent-permanent service as a Health Systems Specialist 2 (SW) OR a current NYS employee with one year or more of permanent or contingent-permanent service as a Health Systems Specialist 2 (SW), G-20 OR eligible for transfer under Section 70.4 of the Civil Service Law (NYS employee with one year of permanent service at G-18 or higher and meeting the open competitive minimum qualification listed below).

Open Competitive Qualifications for 70.4 Transfer and Provisional Consideration: current licensure as either a Licensed Master Social Worker or Licensed Clinical Social Worker with the New York State Education Department and one year of post-master's clinical experience in a licensed and/or certified health facility or program* OR utilizing clinical knowledge in the review or oversight of clinical services for quality assurance or reimbursement purposes. * Examples of qualifying health facilities and programs include hospitals, nursing homes, adult homes, ambulatory care facilities, managed care organizations, psychiatric centers, developmental centers, diagnostic and treatment centers, home health care agencies, assisted living facilities, alcoholism and substance abuse treatment facilities, and community-based agencies that serve the developmentally disabled. Experience as a private practitioner or in a physician’s office is NOT considered qualifying.

Preferred Qualifications: Strong knowledge base in the areas of including resident assessments, knowledge of DSMV, use of psychotropic medications in geriatric and long-term care setting Strong written and verbal communication skills. Excellent interpersonal and organizational skills. Ability to work well independently and as part of a team. Long Term Care experience is a plus. Strong computer and typing skills.

Duties Description Conduct on-site surveys and focused infection control inspections independently or as part of an interdisciplinary team by observing and evaluating resident care against established standards and to determine provider compliance with federal, state and department regulations and policies; Interview residents, representatives and staff and review records; Investigate incidents and resident complaints; Serve as subject matter expert for other survey staff to determine if facilities are meeting the psycho-social needs and problems of residents. Draft Statements of Deficiencies, Plans of Correction, investigative reports, and survey information against Statewide standards for consistency and quality assurance; Review provider emergency plans; Participate in other program-related activities as needed.

Additional Comments Permanent or provisional pending examination, full-time. Provisional candidates will be required to take the next holding of the Civil Service HSS 2 (Social Work) examination and must be eligible for immediate appointment from the resultant eligible list to maintain continued employment as an HSS 2 (Nutrition). Candidates considered for this position must have Current registration as a Social Worker (SW) with the New York State Education Department must be maintained for continued employment in title. Candidates must pass SMQT within first year of employment, as required by the Centers for Medicare and Medicaid Services to conduct surveys. Routine travel, including travel to areas that may not be served by public transportation, is required. Plus additional $3,026 location pay
